We have made available to you the necessary forms required by the funeral home and crematory along with legal forms required by state, county and city officials for the burial or cremation of the decedent's remains. These forms are all in PDF format which allows you to fill in the information online and then print at your local printer. You would then bring these forms with you during the arrangement conference.

Burial Forms

  1. Vital Statistics For Death Certificate ›
    This form is required Death Certificate.
  2. Authorization To Accept or Decline Embalming ›
    This form is required to give authorization to embalm or not embalm the body of the deceased.
  3. Disclosure of Preneed Funeral Agreement ›
    This form is required to acknowledge that any preneed instructions were presented. Pre-Need documents are written instruction regarding goods or services or both goods and services for final disposition of human remains when the goods or services are not provided until the time of death.
  4. Order to Release Remains ›
    This is a county form required to have the medical examiner release the remains of the deceased to the funeral home or crematory.

Cremation Forms

    This form is required to declare how the remains of the deceased are cared for.
